JAMAのLetterから。インフルエンザワクチンについて実務的なことがまとまっている。非常に多くの医師が関わる可能性があるため行う場合は目を通しておくとよいかも。
僕は全然畑違いで新鮮な内容でした。面白かったのは米国で使用可能なワクチンが不活化以外に色々な種類があること。recobinant inactivated vaccineは重度の卵アレルギーでも使えそうでいいなと思いました。
以下本文抜粋
・禁忌がなければ生後6ヶ月以上であればインフルエンザAとBに対する予防接種が毎年全例に推奨される
→米国で手に入るインフルエンザワクチン
・timing
→USでは10月の終わりまでに提供されインフルエンザが蔓延している限り提供されるべき。
→ほとんどの成人で血中の抗体のピークは摂取2週間後。早期の予防接種（7or8月）はインフルエンザシーズンの終盤でsuboptimal immunityになるかも。特に高齢者で
→小児で2回必要な場合はfirst doseはなるべく早く。2回目は10月の終わりまでに。
・Live-attenuated vaccine→FluMist Quadrivalent。FDAでは健康な非妊婦の2-49歳が対象
・Recombinant vaccine→Flublok Quadrivalent。influenza virus or chicken eggsなしで作成されている。一般的なワクチンの3倍量の抗原を含んでいる
・High-Dose Vaccine→Fluzone High-Dose。普通の4倍量の抗原。65歳以上の成人31989人を対象としたRCTではhigh-dose vaccineが有意にgreater antibody responsesがあり、laboratory-confirmed influenza illnessの予防において24.2%より効果的（ N Engl J Med. 2014;371(7):635-645.）
・pregnancy→The ACIP and the American College of Obstetricians and Gynecolo- gists recommend vaccinating pregnant women against influenza without regard to the trimester of pregnancy, but they should not receive the live-attenuated vaccine.
・Allegy
→The ACIP, the American Academy of Allergy, Asthma and Immu- nology, and the American College of Allergy, Asthma and Immunology state that any age-appropriate influenza vaccine can be administered to persons who report a history of hives related to egg exposure.
→Persons with more severe egg allergy can also receive any age-appropriate influenza vaccine, but they should be vaccinated in a healthcare setting with supervision by a health- care provider experienced in recognizing and managing severe allergic reactions.
・immunocompromised persons→live-attenuated influenza vaccinesは避けるべき
・Adverse Effects
→Guillain-Barre syndromeと関連があると言われているが絶対リスクは低い（1-2 additional cases per million persons vaccinated）
→注射部の痛みを除けばほとんどない。
→live-attenuated vaccineに関連したよくあるadverse reactionsは runny nose, nasal congestion, fever, and sore throat。
